Development of Health Care Model for Homebound Elderly by Community Involvement in Nhong-seng Village, Tambon Wang-nang, Muang District, Maha Sarakham Province

Abstract

This study is an action research. The purpose of this study was to investigate the development of the health care model of the elderly in Baan Nong Seng community, Mueang District, Maha Sarakham Province. By the concept of participation. A sample of this study was selected purposively including 20 stakeholders and 31 homebound elderly people. Data were collected by quantitative and qualitative approaches. Then, data were analyzed by descriptive statistics such as, frequency, percentage, mean, standard deviation and qualitative data used content analysis.

The results showed that the development process consisted of 5 phases: 1) context analyses, 2) operating action plans, 3) operating activities, 4) supervision the activities 5) evaluation and after action reviews form to the model of health care for homebound elderly people with a community involvement by WANG NANG Model to composed of W (We), all of us in all sectors A (Attitude) to modify good attitude towards for elderly people N (Non-communicable disease: NCD) Reduction of complications from chronic non-communicable disease G (Giver) Care for the elderly from the family and people in the community N (Network) Network. Co-operate through all communication channels. A (Appreciate). Positive Energy N (Nursing Process) Nursing Care for the Elderly in the community. G: (Good Governance) Good management fair share and responsible for duty.

The factors contributing to this success are the strong community leaders, the local government, the budget supporters, and the main host. District Health Promotion Hospital Support and mentor People in the community cooperate all processes. Continuous and informal coordination.

การศึกษาครั้งนี้เป็นการวิจัยเชิงปฏิบัติการ มีความมุ่งหมายเพื่อศึกษากระบวนการพัฒนารูปแบบการดูแลสุขภาพผู้สูงอายุกลุ่มติดบ้านโดยการมีส่วนร่วมของชุมชนบ้านหนองเส็ง ตำบลแวงน่าง อำเภอเมือง จังหวัดมหาสารคาม โดยใช้แนวคิดการมีส่วนร่วม กลุ่มตัวอย่างคัดเลือกแบบเจาะจง ประกอบด้วย บุคคลที่มีส่วนเกี่ยวข้อง 20 คน และผู้สูงอายุกลุ่มติดบ้าน 31 คน เก็บรวบรวมข้อมูลทั้งเชิงปริมาณและเชิงคุณภาพ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ การแจงแจงความถี่ จำนวน ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐานและวิเคราะห์ข้อมูลเชิงคุณภาพ โดยการวิเคราะห์เชิงเนื้อหา

ผลการวิจัย พบว่า กระบวนการพัฒนา ประกอบด้วย 5 ขั้นตอน คือ 1) ศึกษาวิเคราะห์บริบทพื้นที่ 2) การวางแผนเชิงปฏิบัติการ 3) ปฏิบัติตามแผน 4) นิเทศติดตาม 5) สรุปผลและถอดบทเรียน เกิดรูปแบบการดูแลสุขภาพผู้สูงอายุกลุ่มติดบ้านโดยการมีส่วนร่วมของชุมชน ภายใต้แนวคิด WANG NANG Model ประกอบไปด้วย W (We) คือพวกเราทุกคน ทุกภาคส่วน A (Attitude) การปรับเปลี่ยนทัศนคติที่ดีต่อผู้สูงอายุ N (Non- communicable disease: NCD) การลดภาวะ แทรกซ้อนจากโรคไม่ติดต่อเรื้อรัง G (Giver) การให้การดูแลผู้สูงอายุทั้งจากคนในครอบครัว ลูกหลานและคนในชุมชน N (Network) เครือข่ายร่วมปฏิบัติงานผ่านทุกช่องทางการติดต่อ สื่อสาร A (Appreciate) การเสริมพลังทางบวก N (Nursing Process) การปฏิบัติการพยาบาลในการดูแลผู้สูงอายุในชุมชน G :(Good Governance) การบริหารจัดการที่ดี มีความรับผิดชอบต่อหน้าที่ มีส่วนร่วมอย่างเป็นธรรม

โดยปัจจัยที่ทำให้เกิดความสำเร็จในครั้งนี้ ประกอบด้วย มีผู้นำชุมชนดี เข้มแข็ง องค์กรปกครองส่วนท้องถิ่นสนับสนุนงบประมาณและเป็นเจ้าภาพหลัก โรงพยาบาลส่งเสริมสุขภาพตำบล สนับสนุนและเป็นพี่เลี้ยง ประชาชนในชุมชนให้ความร่วมมือทุกกระบวนการ และการติดต่อประสานงานอย่างต่อเนื่องทั้งเป็นทางการและไม่เป็นทางการ
