The Model of Control and Prevention of Liver Fluke Disease Ban yuat Sub-district Sangkhom District Udon Thani Province

Abstract

This action research was aimed to developing process a model for control and prevention of liver fluke disease in the community of Banyuat Sub-District, Sangkhom District, Udon Thani Province. The paticipants were selected from selection criteria of 75 peoples. Data was collected both quantitative and qualitative. Quantitative data was analyze using by frequency, percentage, mean, standard and Paired Sample t-test. Qualitative analyzed by content.

The research found that the development of this model has 11 stages: 1) Analysis of the context and situation 2) Workshop, 3) The Health Statute, 4) Draft Health Statute, 5) Improve the draft Health Statute, 6) Annouce used for Health Statute, 7) Training program for volunteer guinea pig liver fluke, 8) Supervision and follow up, 9) Evaluation, 10) Knowledge, 11) Lessons learned for key sucess factors. The results found that had and everage score of knowledge, role, participation, behavior, operation satisfaction, control and prevention of liver fluke disease increased significantly (p-value<0.001). As a result, the basic model for control and prevention of liver fluke disease in the community is PROTECT Model

In conclusions, the key success factors were development of a model. As a result, three principlr is D-H-B 1) Development: D is mean the development, participation and acceptance of process for people involved 2) Harmony: H is mean the harmony and several elements for control and prevention in community 3) Best Behavior: B is mean the empowerment for best behavior of people.

การศึกษาวิจัยเชิงปฏิบัติการครั้งนี้ มีวัตถุประสงค์ เพื่อศึกษากระบวนการพัฒนารูปแบบในการดำเนินงานควบคุมและป้องกันโรคพยาธิใบไม้ตับในชุมชน ตำบลบ้านยวด อำเภอสร้างคอม จังหวัดอุดรธานี ใช้กลุ่มตัวอย่าง จำนวน 75 คน เก็บรวบรวมข้อมูลทั้งเชิงปริมาณและเชิงคุณภาพ วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติเชิงพรรณา ได้แก่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติเชิงอนุมาน คือ Paired Sample t-test วิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์เนื้อหา

ผลการวิจัย พบว่า กระบวนการพัฒนารูปแบบในการดำเนินงานควบคุมและป้องกันโรคพยาธิใบไม้ตับในชุมชนครั้งนี้ ประยุกต์แนวคิดกระบวนการธรรมนูญสุขภาพ ทำให้เกิดกระบวนการ 11 ขั้นตอน ได้แก่ 1) วิเคราะห์บริบทและสถานการณ์ 2) ประชุมเชิงปฏิบัติการ 3) ยกร่างธรรมนูญสุขภาพ 4) เวทีพิจารณาร่างธรรมนูญสุขภาพ 5) ปรับปรุงร่างธรรมนูญและจัดเวทีรับฟังความคิดเห็น 6) ประกาศใช้ธรรมนูญสุขภาพ 7) ดำเนินโครงการอบรมแกนนำอาสาสมัครพิทักษ์พยาธิใบไม้ตับ 8) นิเทศและติดตาม 9) ประเมินผล 10) การแลกเปลี่ยนเรียนรู้ 11) ถอดบทเรียนเพื่อหาปัจจัยแห่งความสำเร็จ ผลตามกระบวนการดังกล่าว ส่งผลให้กลุ่มเป้าหมายมีคะแนนเฉลี่ยด้านความรู้ ด้านบทบาทหน้าที่ ด้านการมีส่วนร่วม ด้านพฤติกรรมการปฏิบัติตน และด้านความพึงพอใจในการดำเนินงานควบคุมและป้องกันโรคพยาธิใบไม้ตับเพิ่มขึ้น อย่างมีนัยสำคัญทางสถิติ (p-value<0.001) และยังส่งผลให้เกิดรูปแบบเบื้องต้นในการดำเนินงานควบคุมและป้องกันโรคพยาธิใบไม้ตับในชุมชน คือ PROTECT Model

สรุป ปัจจัยแห่งความสำเร็จในครั้งนี้ เป็นผลมาจากหลักสามประการคือ D-H-B กล่าวคือ Development: D หมายถึง การพัฒนาโดยอาศัยการมีส่วนร่วมและการยอมรับร่วมกันของผู้เกี่ยวข้องในทุกขั้นตอน Harmony: H หมายถึง ความสามัคคีและความสอดคล้องกันขององค์ประกอบต่างๆในการขับเคลื่อนการดำเนินงานในพื้นที่ Best Behavior: B หมายถึง การเสริมพลังให้ประชาชนมีการปฏิบัติตนที่ดี และถูกต้องอย่างต่อเนื่อง
