Development of community care model for pulmonary tuberculosis by the Health Network of Thatpanom District, Nakhon Phanom Province

Abstract

This action research objective to study the process of developing care for pulmonary tuberculosis patients in the community by the That Phanom District Health Network Nakhon Phanom Province, to reduce the rate of drug deficiency in patients with pulmonary tuberculosis, new fungi were found Thatphanom Crown Prince Hospital, That Phanom District. Target group of 97 people is a patient with pulmonary tuberculosis caregivers of pulmonary tuberculosis patients, Community health networks include community leaders and village health volunteers, and the staff responsible for tuberculosis are tuberculosis clinic, staff Tambon Health Promoting Hospital, That Phanom City Community Health Center, District Public Health Office and Local Administrative Organization Officers. Methods of study quantitative and qualitative data were collected. using an interview and observation form. participation in activities, Data were analyzed using descriptive statistics such as frequency, percentage and standard deviation. The process of studying the development process for pulmonary tuberculosis patients in the community by the That Phanom District Health Network Nakhon Phanom Province There are 7 steps of operation, 1) Analyze the situation and problems of caring for pulmonary tuberculosis patients in the community, 2) meet relevant parties to develop and formulate an action plan, 3) formulate a plan for caring for pulmonary tuberculosis patients in the community by health networks, 4) implement an action plan, 5) follow-up assessment 6) reflect the performance according to the action plan and 7) analyze what needs to be improved.

Results caring for tuberculosis patients in That Phanom District Operated by the tuberculosis committee at that Phanom district level. Provided by a multidisciplinary team. Pulmonary tuberculosis patients are cared for by the health network, including public health officials, Village Health Volunteer, Local Administrative Organization and community leaders, according to the context and daily lifestyle of the patient. There were no drug-deficient. The performance showed no drug-deficient pulmonary tuberculosis patients.All pulmonary tuberculosis patients had a mentor to direct the medication intake. 100.00% of pulmonary tuberculosis patients and caregivers of pulmonary tuberculosis patients. There was a high level of satisfaction in caring for pulmonary tuberculosis patients in the community of That Phanom Health Network (average = 4.33, standard deviation =0.28).

Conclusion The process of developing care for pulmonary tuberculosis patients in the community by the That Phanom District Health Network Nakhon Phanom Province, There is a success factor, Community-based pulmonary tuberculosis patients are cared for by health networks, by caring for a relative relationship It is consistent with the lifestyle of pulmonary tuberculosis patients, Getting help from relevant agencies in the community And there is a way to get advice from experts. This results in a decrease in the rate of drug withdrawal in pulmonary tuberculosis patients.

การวิจัยเชิงปฏิบัติการ (action research) ครั้งนี้มีวัตถุประสงค์เพื่อศึกษากระบวนการพัฒนาการดูแลผู้ป่วยวัณโรคปอดในชุมชน โดยเครือข่ายสุขภาพอำเภอธาตุพนม จังหวัดนครพนม ในการลดอัตราการขาดยาของผู้ป่วยวัณโรคปอดเสมหะพบเชื้อรายใหม่ โรงพยาบาลสมเด็จพระยุพราชธาตุพนม อำเภอธาตุพนม กลุ่มเป้าหมาย จำนวน 97 คน คือ ผู้ป่วยวัณโรคปอด ผู้ดูแลผู้ป่วยวัณโรคปอด เครือข่ายสุขภาพในชุมชน ได้แก่ ผู้นำชุมชนและอาสาสมัครสาธารณสุขประจำหมู่บ้าน และเจ้าหน้ารับผิดชอบด้านวัณโรค ได้แก่ เจ้าหน้าที่คลินิกวัณโรค โรงพยาบาลส่งเสริมสุขภาพตำบล ศูนย์สุขภาพชุมชนเมืองธาตุพนม สำนักงานสาธารณสุขอำเภอและเจ้าหน้าที่องค์กรปกครองส่วนท้องถิ่น วิธีการศึกษา เก็บรวบรวมข้อมูลเชิงปริมาณและเชิงคุณภาพ โดยใช้แบบสัมภาษณ์และแบบสังเกต การเข้าร่วมกิจกรรม วิเคราะห์ข้อมูลโดยใช้ สถิติพรรณนา ได้แก่ ความถี่ ร้อยละ และส่วนเบี่ยงเบนมาตรฐาน การศึกษากระบวนการพัฒนาการดูแลผู้ป่วยวัณโรคปอดในชุมชน โดยเครือข่ายสุขภาพอำเภอธาตุพนม จังหวัดนครพนม มีขั้นตอนการดำเนินงาน 7 ขั้นตอน ดังนี้ 1) วิเคราะห์สถานการณ์และสภาพปัญหาการดูแลผู้ป่วยวัณโรคปอดในชุมชน 2) ประชุมผู้เกี่ยวข้องเพื่อพัฒนากำหนดแผนปฏิบัติการ 3) กำหนดแผนการดูแลผู้ป่วยวัณโรคปอดในชุมชนโดยเครือข่ายสุขภาพ 4) ปฏิบัติงานตามแผนปฏิบัติการ 5) ประเมินติดตามผล 6) สะท้อนผลการปฏิบัติงานตามแผนปฏิบัติการ และ 7) วิเคราะห์สิ่งที่ต้องพัฒนาปรับปรุง

ผลการศึกษา พบว่า การดูแลผู้ป่วยวัณโรคอำเภอธาตุพนม ดำเนินงานโดยคณะกรรมการวัณโรคระดับอำเภอธาตุพนม ให้บริการโดยทีมสหวิชาชีพ ผู้ป่วยวัณโรคปอดในชุมชน ได้รับการดูแลโดยเครือข่ายสุขภาพ ได้แก่ เจ้าหน้าที่สาธารณสุข อาสาสมัครสาธารณสุขประจำหมู่บ้าน องค์การบริหารส่วนท้องถิ่น และผู้นำชุมชน ตามบริบทและวิถีชีวิตประจำวันของผู้ป่วย ผลการดำเนินงานไม่พบผู้ป่วยวัณโรคปอดขาดยา ผู้ป่วยวัณโรคปอดทุกรายมีพี่เลี้ยงกำกับการรับประทานยา ร้อยละ 100.00 ผู้ป่วยวัณโรคปอดและผู้ดูแลผู้ป่วยวัณโรคปอด มีความพึงพอใจในการดูแลผู้ป่วยวัณโรคปอดในชุมชนของเครือข่ายสุขภาพอำเภอธาตุพนม ในระดับสูง ค่าเฉลี่ยเท่ากับ 4.33 ส่วนเบี่ยงเบนมาตรฐานเท่ากับ 0.28

สรุปผล กระบวนการพัฒนาการดูแลผู้ป่วยวัณโรคปอดในชุมชน โดยเครือข่ายสุขภาพอำเภอธาตุพนม จังหวัดนครพนม มีปัจจัยความสำเร็จคือ ผู้ป่วยวัณโรคปอดในชุมชนได้รับการดูแลจากเครือข่ายสุขภาพ โดยการดูแลที่มีความสัมพันธ์แบบญาติพี่น้อง มีความสอดคล้องกับวิถีการดำเนินชีวิตของผู้ป่วยวัณโรคปอด การได้รับความช่วยเหลือจากหน่วยงานที่เกี่ยวข้องในชุมชน และมีช่องทางในการขอรับคำปรึกษาจากผู้เชี่ยวชาญ ทำให้อัตราการขาดยาของผู้ป่วยวัณโรคปอดลดลง
