Quality Improverment Model of In-Patient Medical Records by Interdisciplinary Team in Det Udom Crow Prince Hospital, Det Udom District, Ubonratchathani Province

Abstract

This action research explored for the quality improvement of in-patient medical records by interdisciplinary team in Det Udom crown prince hospital, Det Udom district, Ubonratchatani province. The research was conducted between March 2018 to July 2018. The sample consisted of 104 workers were involved the recording of In-patient medical records using multi-stage random sampling. The data collected was both quantitatively and qualitatively. Using the questionnaires, conference record, group discussion, observation record, in-depth interviews and SWOT analysis record. The analysis of quantitative data using descriptive statistic such as frequency, percentage, mean, standard deviation and inferential statistic as paired simple t-test. The analysis of qualitatively data using content analysis.

The research found that the development process has 6 stages: 1) Problem analysis 2) Appointment of the working group 3) Plan to action 4) Implementation of the action plan 5) Evaluate the development and 6) Reflecting and lessons to find success factors. That the development process causes the quality of in-patient medical records and the practices in recording of interdisciplinary team were high after the development of program satisfaction increased significantly (p-value < 0.05).

According to the conclusion of the key succeeded factors of connecting each other’s in the workplace as family, executive support, clearly policies and supervision system.

การวิจัยการวิจัยเชิงปฏิบัติการครั้งนี้มี วัตถุประสงค์เพื่อศึกษาการพัฒนาคุณภาพระบบการบันทึกเวชระเบียนผู้ป่วยใน ของทีมสหวิชาชีพ โรงพยาบาลสมเด็จพระยุพราชเดชอุดม อำเภอเดชอุดม จังหวัดอุบลราชธานี ดำเนินการวิจัยระหว่างเดือนมีนาคม – กรกฎาคม พ.ศ. 2561 คัดเลือกกลุ่มเป้าหมายโดยการสุ่มแบบหลายขั้นตอน จำนวน 104 คน เก็บข้อมูลทั้งเชิงปริมาณและเชิงคุณภาพ โดยใช้แบบสอบถาม, แบบบันทึกการประชุม, แบบสนทนากลุ่ม, แบบสังเกต, แบบสัมภาษณ์เชิงลึก และแบบวิเคราะห์ศักยภาพองค์กร วิเคราะห์ข้อมูลเชิงปริมาณ ด้วยสถิติเชิงพรรณนาได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติเชิงอนุมาน ได้แก่ Paired simple t-test วิเคราะห์ข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เชิงเนื้อหา

ผลการวิจัย พบว่า กระบวนการพัฒนาครั้งนี้ มี 6 ขั้นตอน ประกอบด้วย 1) ศึกษาบริบทและวิเคราะห์ปัญหา 2) แต่งตั้งคณะทำงาน 3) จัดทำแผนปฏิบัติการ 4) ดำเนินการตามแผนปฏิบัติการ 5) ประเมินผลการพัฒนา และ 6) คืนข้อมูลและถอดบทเรียนหาปัจจัยแห่งความสำเร็จ กระบวนการดังกล่าวส่งผลให้ ผลการประเมินคุณภาพการบันทึกเวชระเบียนผู้ป่วยใน และพฤติกรรมในการบันทึกเวชระเบียนผู้ป่วยใน ของทีมสหวิชาชีพ มีการเปลี่ยนแปลงค่าเฉลี่ยดีกว่าก่อนการดำเนินการพัฒนา อย่างมีนัยสำคัญทางสถิติ (p-value < 0.05)

โดยสรุป ปัจจัยแห่งความสำเร็จครั้งนี้ เกิดจากการร่วมกันทำงาน การสร้างวัฒนธรรมการทำงานแบบพี่สอนน้อง มีการสนับสนุนของผู้บริหารโดยกำหนดนโยบายคุณภาพในการทำงาน และการนิเทศ ติดตาม ให้กำลังใจอยู่เสมอ
