Quality Improvement Model for Laboratory Standard in Sub-district Health Promoting Hospital in Chaturapakphiman, Roi-Et

Abstract

Laboratory analysis in the Sub-district Health Promoting Hospital-SHPH is one of the necessary activities. It was called the quality standard of the SHPH (5 Stars Rating) to ensure the quality of service in the office. This action research aimed to developing models of quality improvement in medical care and public health laboratories in SHPH network of Chaturapakphiman district, Roi-Et province. The 35 targets group was selected as the key informants. Both quantitative and qualitative data were collected by observing interview and questionnaire including self-assessment questions. Data were analyzed by descriptive statistics such as frequency, percentage, mean, standard deviation. It was applying content analysis method for qualitative data analysis.

The study indicated that the development process consists of 7 steps: 1) Appointment of task force committee 2) Situational analysis 3) Formulation of action plan 4) Plan Implementation 5) Observation 6) Data Retrieval and Comparison 7) Summary and Lesson learned. After the operation, it was found that the participant’s knowledge scores in practice were increased from 42.80% to 91.40%. Participation’s level was increased from 25.70% to 65.70% and the satisfaction of the process was at the high level (mean = 4.05, SD = 0.32). Additionally, all SHPHs were passed with regards to the criterion of the 5 stars rating standard.

In summary, the key significantly success of this process comprised of 1) clearly and direction of policy from the top administrators, 2) participation of a multidisciplinary team and 3) the effectively of monitoring and supportive system among the network.

การตรวจวิเคราะห์ทางห้องปฏิบัติการในโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) เป็นหนึ่งในกิจกรรมที่จำเป็น ที่ต้องมีคุณภาพตามมาตรฐานโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาว (รพ.สต.ติดดาว) เพื่อให้เกิดความมั่นใจต่อการใช้บริการ การวิจัยครั้งนี้เป็นการวิจัยเชิงปฏิบัติการ (Action Research) มีความมุ่งหมายเพื่อพัฒนารูปแบบการพัฒนาคุณภาพห้องปฏิบัติการด้านการแพทย์และสาธารณสุข ตามเกณฑ์มาตรฐานโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาวในเครือข่ายหน่วยบริการปฐมภูมิ อำเภอจตุรพักตรพิมาน จังหวัดร้อยเอ็ด กลุ่มเป้าหมายในการวิจัยประกอบด้วยผู้เกี่ยวข้อง จำนวน 35 คน เก็บรวบรวมข้อมูลโดยการสังเกต การสัมภาษณ์ โดยใช้แบบสอบถาม และประเมินตนเอง และวิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน และการวิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์เชิงเนื้อหา

ผลการศึกษาพบว่า กระบวนการพัฒนาครั้งนี้ประกอบด้วยขั้นตอน 7 ขั้นตอนได้แก่ 1) การแต่งตั้งคณะทำงาน 2) การวิเคราะห์สถานการณ์ 3) การจัดทำแผนปฏิบัติการ 4) การดำเนินงานตามแผน 5) สังเกตการดำเนินงาน 6) คืนข้อมูลและเปรียบเทียบผล และ 7) สรุปและถอดบทเรียน และพบว่าภายหลังการดำเนินงาน ในส่วนเจ้าหน้าที่สาธารณสุขมีคะแนนความรู้อยู่ในระดับสูง โดยมีคะแนนเพิ่มจากร้อยละ 42.80 เป็นร้อยละ 91.40 การมีส่วนร่วมคะแนนเพิ่มจากร้อยละ 25.70 เป็นร้อยละ 65.70 และความพึงพอใจอยู่ในระดับมาก (mean =4.05, SD.=0.32) ภายหลังการพัฒนา พบว่า โรงพยาบาลส่งเสริมสุขภาพตำบล ผ่านตามเกณฑ์มาตรฐานโรงพยาบาลส่งเสริมสุขภาพตำบลติดดาวทุกแห่ง

โดยสรุปกระบวนการพัฒนาคุณภาพครั้งนี้ มีปัจจัยแห่งความสำเร็จที่สำคัญ คือ การกำหนดนโยบายจากประธานเครือข่ายที่ชัดเจน การมีส่วนร่วมในการพัฒนาของทีมสหวิชาชีพ และการมีระบบกำกับ นิเทศ ติดตามของทีมสนับสนุนที่มีประสิทธิภาพ
