Develop a System to Preparation of Kidney Transplantation Recipients, Sunpasitthiprasong Hospital

Abstract

End-stage chronic kidney disease is still an important public health problem in the country and in Health Service Area 10, with applicants waiting for kidney donations on the rise. Preparing patients before waiting for a kidney transplant is therefore very important. This study aims to develop a system for preparing applicants to wait for a kidney transplant at Sunpasitthiprasong Hospital.Using a mixed methods research study in 2 phases, namely the situation study phase and the development phase of the system for preparing applicants waiting for a kidney transplant. Sample groups include: There are 30 patients with end-stage chronic kidney disease who need a kidney transplant at Sunpasitthiprasong Hospital and a multidisciplinary team that performs organ transplants, 20 people.The research tools used were interviews and questionnaires for multidisciplinary teams. Questionnaires for patients include data recording forms. General information questionnaire Quality of life questionnaire Test of knowledge about self-care Statistics used in the analysis are frequency, percentage, mean, standard deviation.

Results of the study: Situation and operational context for patients with end-stage renal disease who wish to apply and wait.Received a kidney transplant and found that the service was quite good. It is well equipped with personnel, medicine, medical supplies and information systems. But screening lacks continuous assessment of health status. The work preparation coordination system lacks connections between each agency. After developing a quality of life system for applicants waiting to receive a kidney transplant. Overall is at a good level. Good knowledge about self-care after developing a system for preparing candidates waiting to receive a kidney transplant. Overall at a good level Satisfaction with the use of the system for preparing applicants to wait for a kidney transplant. Overall it is at a high level. The developed evaluation process for preparing applicants to wait for a kidney transplant still has 5 steps, reducing the number of tests to 7: These include: 1) Evaluation of suitability to wait for a kidney transplant by a doctor. 2) Readiness assessment Providing advice about kidney transplantation 3) Assessment and checking of physical and mental readiness 4) Summary of the results of the assessment of physical and mental readiness. 5) Evaluate the risk of kidney rejection reactions in patients who wish to register to receive a kidney transplant.

โรคไตเรื้อรังระยะสุดท้ายยังเป็นปัญหาด้านสาธารณสุขที่สำคัญของประเทศและในพื้นที่เขตบริการสุขภาพที่ 10 โดยมีผู้สมัครรอรับบริจาคไตแนวโน้มสูงขึ้น การเตรียมตัวผู้ป่วยตั้งแต่ก่อนเข้ารอการปลูกถ่ายไตจึงมีความสำคัญเป็นอันมาก การศึกษานี้มีวัตถุประสงค์เพื่อพัฒนาระบบการเตรียมผู้สมัครเพื่อรอรับการปลูกถ่ายไตโรงพยาบาลสรรพสิทธิประสงค์ ใช้การศึกษาวิจัยแบบผสมผสานวิธี 2 ระยะ ได้แก่ ระยะศึกษาสถานการณ์ และระยะพัฒนาระบบการเตรียมผู้สมัครรอปลูกถ่ายไต กลุ่มตัวอย่างได้แก่ ผู้ป่วยโรคไตเรื้อเรื้อรังระยะสุดท้ายที่ต้องการปลูกถ่ายไตโรงพยาบาลสรรพสิทธิประสงค์จำนวน 30 คน และ ทีมสหวิชาชีพที่ดำเนินงานปลูกถ่ายอวัยวะจำนวน 20 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสัมภาษณ์ แบบสอบถามสำหรับทีมสหวิชาชีพ แบบสอบถามสำหรับผู้ป่วย ได้แก่ แบบบันทึกข้อมูล แบบสอบถามข้อมูลทั่วไป แบบสอบถามคุณภาพชีวิต แบบทดสอบความรู้เกี่ยวกับการดูแลตนเอง สถิติที่ใช้ในการวิเคราะห์คือ ความถี่ ร้อยละ ค่าเฉลี่ย ค่าส่วนเบี่ยงเบนมาตรฐาน

ผลการศึกษา สถานการณ์การดำเนินงานผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ต้องการสมัครรอรับการปลูกถ่ายไต พบว่า ด้านการให้บริการค่อนข้างดี มีความพร้อมทั้งบุคลากร ยา เวชภัณฑ์และระบบสารสนเทศ แต่การคัดกรองขาดการประเมินภาวะสุขภาพอย่างต่อเนื่อง ระบบประสานงานการเตรียมงานขาดความเชื่อมโยงของแต่ละหน่วยงาน ภายหลังพัฒนาระบบคุณภาพชีวิตผู้สมัครเพื่อรอรับการปลูกถ่ายไต โดยรวมอยู่ในระดับดี ความรู้เกี่ยวกับการดูแลตนเองหลังพัฒนาระบบการเตรียมผู้สมัครรอรับการปลูกถ่ายไต โดยรวมในระดับดี ความพึงพอใจต่อการใช้บริการระบบการเตรียมผู้สมัครเพื่อรอรับการปลูกถ่ายไต โดยรวมอยู่ในระดับมาก ความพึงพอใจของผู้ให้บริการต่อการพัฒนาระบบการเตรียมผู้สมัครเพื่อรอรับการปลูกถ่ายไต โดยรวมอยู่ในระดับมากที่สุด ขั้นตอนการประเมินเตรียมผู้สมัครรอรับการปลูกถ่ายไตที่พัฒนาขึ้น ยังคงมี 5 ขั้นตอน ลดจำนวนที่ตรวจเหลือ 7 ครั้ง ได้แก่ 1) การประเมินความเหมาะสมรอรับการปลูกถ่ายไตโดยแพทย์พิจารณา 2) การประเมินความพร้อม ให้คำปรึกษาเกี่ยวกับการปลูกถ่ายไต 3) การประเมินตรวจสอบความพร้อมทั้งร่างกายและจิตใจ 4) สรุปผลการประเมินความพร้อมทั้งด้านร่างกายและจิตใจ 5) ประเมินความเสี่ยงของการเกิดปฏิกิริยาการปฏิเสธไตในผู้ป่วยที่ต้องการขึ้นทะเบียนรอรับการปลูกถ่ายไต

สรุป ระบบการเตรียมผู้สมัครรอรับการปลูกถ่ายไตที่พัฒนาขึ้นนี้ ช่วยให้การดำเนินงานมีความชัดเจนและเป็นรูปธรรม มีการคัดกรองอย่างครอบคลุม การประเมินภาวะสุขภาพอย่างต่อเนื่อง เพิ่มการเข้าถึงบริการ และเกิดการมีส่วนร่วมจากผู้เกี่ยวข้องทุกระดับ
