The Integrated Cooperative Improvement of Locality Organizations to LTC Fund Administrative System in Nongbuadaeng district Chaiyaphumi Province

Abstract

This action research aimed to examine the integration and cooperation of area agencies to manage in Long Term Care Fund at Nong Bua Daeng municipality, Nong Bua Daeng District, Chaiyaphum Province. The participants were included of Group 1 as 10 representatives of related organizations or agencies in the area, Group 2 as a group of people, namely 1) 60 dependent person, 2) 20 relatives or caregivers of dependent person, Group 3, the service provider group such as 1) 2 Care managers: CMs, 2) 20 Care-givers: CGs, 3) 48 Village health volunteers, totaling 160 person. Quantitative data was collected using a questionnaire created. Quantitative data was applied a descriptive statistics including percentage, mean, standard deviation and inferential statistic such t-test. Qualitative data were analyzed using content analysis.

The results of the study found that this process were comprised 8 steps as follows: 1) situation and context analyses, 2) arrange a meeting to plan, 3) request approval of the plan, 4) implementation, 5) monitoring, 6) evaluation, 7) summarize the results and 8) feedback and lessons learned. The results of the action when was comparing before and after found that all 3 target groups that their knowledge and understanding and participation. It was found that there was a statistically significant improvement (p-value 0.05), resulting in the assessment of the evaluation of the LTC fund from being at a fair level (65 points) to being at a very good level (92 points).

In summary, the success factors in this operation include 1) creating a network at the local level 2) strengthening of capacity building their competencies of those involved to increasing skills and ability to work; and 3) devolution power all person to take action in their own areas. That was called a N-B-D model.

การวิจัยเชิงปฏิบัติการครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาการบูรณาการความร่วมมือของหน่วยงานระดับพื้นที่ในการพัฒนาระบบการบริหารจัดการกองทุนผู้สูงอายุที่มีภาวะพึ่งพิงในพื้นที่เทศบาลตำบลหนองบัวแดง อำเภอหนองบัวแดง จังหวัดชัยภูมิ กลุ่มเป้าหมายประกอบด้วย กลุ่มที่ 1 กลุ่มผู้แทนองค์กรหรือหน่วยงานที่เกี่ยวข้องในพื้นที่ จำนวน 10 คน กลุ่มที่ 2 กลุ่มประชาชน คือ 1) ผู้ที่มีภาวะพึ่งพิง จำนวน 60 คน 2) ญาติหรือผู้ดูแลผู้ที่มีภาวะพึ่งพิง จำนวน 20 คน กลุ่มที่ 3 กลุ่มผู้ให้บริการ คือ 1) Care manager: CM จำนวน 2 คน 2) Care-giver: CG จำนวน 20 คน 3) อสม. จำนวน 48 คน รวมทั้งสิ้น 160 คน เก็บข้อมูลเชิงปริมาณโดยใช้แบบสอบถามที่สร้างขึ้น วิเคราะห์ข้อมูลเชิงปริมาณ ด้วยสถิติเชิงพรรณนา ได้แก่ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติเชิงอนุมาณ ได้แก่ t-test วิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์เชิงเนื้อหา

ผลการศึกษา พบว่า กระบวนการครั้งนี้มี 8 ขั้นตอน ดังนี้ 1) วิเคราะห์สถานการณ์และบริบทกองทุน 2) จัดประชุมเพื่อวางแผน 3) ขออนุมัติแผน 4) ดำเนินงานตามแผน 5) ติดตามผล 6) ประเมินผล 7) สรุปผล และ 8) สรุปและถอดบทเรียน ผลจากการดำเนินการเมื่อเปรียบเทียบก่อนและหลัง พบว่า กลุ่มเป้าหมายทั้งสามกลุ่ม มีระดับความรู้ และความเข้าใจการดำเนินงานดีขึ้น และด้านการมีส่วนร่วม พบว่ามีการเปลี่ยนแปลงที่ดีขึ้นอย่างมีนัยสำคัญทางสถิติ (p-value 0.05) ส่งผลให้การประเมินการบริหารกองทุนผู้สูงอายุนี้จากแต่เดิมอยู่ในระดับพอใช้ (65คะแนน) เปลี่ยนอยู่ในระดับดีมาก (91 คะแนน)

โดยสรุปปัจจัยแห่งความสำเร็จในการดำเนินงานครั้งนี้ ได้แก่ 1) การสร้างเครือข่ายในระดับพื้นที่ (Network) 2) การเสริมสร้างให้ผู้เกี่ยวข้องมีสมรรถนะด้านความรู้ ทักษะและความสามารถในการทำงานเพิ่มมากขึ้น (Building capacity) และ 3) การมอบอำนาจให้เป็นผู้ลงมือปฏิบัติจริงในพื้นที่ของตน (Devolution) ซึ่งอาจจะเรียกได้ว่า N-B-D model
