The Development Model for Monitoring and Evaluation of the Sub-district Health Promoting Hospital Transferring to the Mahasarakham Provincial Administration Organization

Abstract

The transfer of public health missions to local administrative organizations play a role in organizing public services and public health management for the benefit of local people. It is an important mechanism to increase efficiency. Providing state health services to reach the people better, faster and to meet the needs of the people in the area. It also provides an opportunity for the public to participate. This research aims to study Development of a model for monitoring and evaluation of the Sub-district Health Promoting Hospital Transferring to the Mahasarakham Provincial Administration Organization. The Study is a mixed methods research (MMR), divided into 2 phases: Phase 1 is a multi-method research, Phase 2 is an action research. The target group is Stakeholders and those involved in monitoring and evaluating the performance of the Sub-district Health Promoting Hospital Transferring to the Mahasarakham Provincial Administration Organization totalling 128 places. Data were collected using questionnaires and record expressing opinions from group discussion. Data were analysed using descriptive statistics; frequency, percentage, average, multiple logistic regressions

There were factors affecting the monitoring and evaluation of the Sub-district Health Promoting Hospital Transferring to the Mahasarakham Provincial Administration Organization. It was found that the variables can participate in predictions including health expenses Service Delivery, Health Workforce, Information System, Medical Products Vaccines and Technologies, Financing and Leadership and Governance. Statistically significant at the 0.01 level, all four variables were able to predict the monitoring and evaluation of the Sub-district Health Promoting Hospital Transferring to the Mahasarakham Provincial Administration Organization was 84.6 %. As for the model for monitoring and evaluation of the Sub-district Health Promoting Hospital Transferring to the Mahasarakham Provincial Administration Organization, it consists of 5 elements: 1) Evaluation team structure, 2) Evaluation guidelines, 3) Tools to use in Assessment 4) Coordination and 5) Participation.

การถ่ายโอนภารกิจด้านสาธารณสุขให้กับองค์กรปกครองส่วนท้องถิ่นมีบทบาทในการจัดระบบบริการสาธารณะและการบริหารจัดการด้านสาธารณสุขเพื่อประโยชน์ของประชาชนในท้องถิ่น เป็นกลไกที่สำคัญที่จะเพิ่มประสิทธิภาพ การให้บริการด้านสุขภาพของรัฐให้เข้าถึงประชาชนได้ดีขึ้น เร็วขึ้น และตรงกับความต้องการของประชาชนในพื้นที่ และยังเป็นการเปิดโอกาสให้ประชาชนได้มีส่วนร่วม การวิจัยนี้ มีวัตถุประสงค์เพื่อศึกษา การพัฒนารูปแบบการกำกับและประเมินผลการดำเนินงานของโรงพยาบาลส่งเสริมสุขภาพตำบลที่ถ่ายโอนให้แก่องค์การบริหารส่วนจังหวัดมหาสารคาม เป็นการวิจัยแบบผสมผสานวิธี แบ่งเป็น 2 ระยะคือ ระยะที่ 1 เป็นการวิจัยแบบสำรวจ ระยะที่ 2 เป็นการวิจัยเชิงปฏิบัติการ กลุ่มเป้าหมายคือ ผู้มีส่วนได้ส่วนเสีย และผู้เกี่ยวข้องในการกำกับและประเมินผลการดำเนินงานของโรงพยาบาลส่งเสริมสุขภาพตำบลที่ถ่ายโอนให้แก่องค์การบริหารส่วนจังหวัดมหาสารคาม จำนวน 128 แห่ง เก็บข้อมูลโดยใช้แบบสอบถาม แบบบันทึกการสนทนากลุ่ม การแสดงความคิดเห็น ส่วนสถิติที่ใช้ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย Multiple logistic regression

ผลการศึกษาปัจจัยที่มีผลต่อการกำกับและประเมินผลการดำเนินงานของโรงพยาบาลส่งเสริมสุขภาพตำบลที่ถ่ายโอนให้แก่องค์การบริหารส่วนจังหวัดมหาสารคาม พบว่า ตัวแปรที่สามารถร่วมทำนาย ประกอบด้วย ระบบบริการ กำลังคนด้านสุขภาพ ระบบข้อมูลข่าวสาร เทคโนโลยีทางการแพทย์ ค่าใช้จ่ายด้านสุขภาพ และภาวะผู้นำและธรรมาภิบาล มีนัยสำคัญทางสถิติที่ระดับ 0.01 ตัวแปรทั้ง 6 ด้าน สามารถร่วมทำนายการกำกับและประเมินผลการดำเนินงานของโรงพยาบาลส่งเสริมสุขภาพตำบลที่ถ่ายโอนให้แก่องค์การบริหารส่วนจังหวัดมหาสารคาม ได้ร้อยละ 84.6 ส่วนรูปแบบการกำกับและประเมินผลการดำเนินงานโรงพยาบาลส่งเสริมสุขภาพตำบลที่ถ่ายโอนให้แก่องค์การบริหารส่วนจังหวัดมหาสารคาม ประกอบด้วย 5 องค์ประกอบ ได้แก่ 1) มีโครงสร้างคณะผู้ประเมิน 2) มีแนวทางการประเมิน 3) มีเครื่องมือที่ใช้ในการประเมิน 4) มีการประสานงาน และ 5) การมีส่วนร่วม

ปัจจัยแห่งความสำเร็จของการกำกับและประเมินผลดำเนินงานของโรงพยาบาลส่งเสริมสุขภาพตำบลที่ถ่ายโอนให้แก่องค์การบริหารส่วนจังหวัดมหาสารคาม ได้แก่ 1) นโยบาย ผู้บริหารให้ความสำคัญ มีโครงสร้างคณะผู้ประเมิน 2) การมีส่วนร่วม มีเป้าหมายร่วมกันและมีส่วนร่วมในการดำเนินงาน และ 3) การประสานงาน มีการประสานการดำเนินงานในขั้นตอน วิธีการ รายละเอียดตัวชี้วัดในการประเมินผล ตั้งแต่การให้ข้อมูล ร่วมคิด ร่วมวิเคราะห์ และมีการบูรณาการการทำงานร่วมกัน
