Development of Prevention Model of Hepatitis B in Khum Ngoen Subdistrict, Mueang District, Yasothon Province

Abstract

Hepatitis B is a major public health issue and one of the leading causes of liver cirrhosis and liver cancer. Individuals born before 1992 are at high risk of hepatitis B infection due to not having received the hepatitis B vaccine under the national immunization program of the Ministry of Public Health.This action research aimed to develop an operational model for the prevention of hepatitis B in KhumNgoen Subdistrict, Mueang District, Yasothon Province. The sample consisted of 25 stakeholders and 71 individuals born before 1992 residing in KhumNgoen Subdistrict, Mueang District, Yasothon Province.The study was conducted between May and October 2024. Both quantitative and qualitative data were collected and analyzed using descriptive statistics. A comparison of pre- and post-intervention mean scores was conducted using the Paired Sample t-test with a significance level set at 0.05.

The development process involved situation and context analysis, action planning, implementation, observation, evaluation, and lesson learned extraction. The developed model consisted of four core activities: (1) screening using questionnaires and fingertip blood testing; (2) establishing a care and referral system for infected individuals; (3) improving access to hepatitis B vaccination for unvaccinated individuals; and (4) fostering community participation through local leaders, abbots, teachers, and village health volunteers to support public awareness and household-level education.Post-implementation results showed a statistically significant increase in knowledge and satisfaction scores (p < 0.05). Three individuals were identified as hepatitis B positive and referred to appropriate care. The success of this model was attributed to active stakeholder participation, effective screening and follow-up, improved public awareness, and strong support from local administrators and agencies. These elements contributed to a systematic and sustainable approach to hepatitis B prevention.

โรคไวรัสตับอักเสบบี เป็นปัญหาสาธารณสุขที่สำคัญ เป็นหนึ่งในสาเหตุของโรคตับแข็งและมะเร็งตับ โดยเฉพาะประชากรที่เกิดก่อนพ.ศ. 2535 มีความเสี่ยงสูงต่อการติดเชื้อไวรัสตับอักเสบบี เนื่องจากไม่ได้รับวัคซีนป้องกันโรคตามแผนการสร้างเสริมภูมิคุ้มกันโรคของกระทรวงสาธารณสุข การวิจัยเชิงปฏิบัติการครั้งนี้มีวัตถุประสงค์เพื่อพัฒนารูปแบบการดำเนินงานป้องกันโรคไวรัสตับอักเสบบีในตำบลขุมเงิน อำเภอเมือง จังหวัดยโสธร กลุ่มตัวอย่างประกอบด้วย กลุ่มผู้มีส่วนได้ส่วนเสีย จำนวน 25 คน และประชาชนที่เกิดก่อน พ.ศ. 2535 ในตำบลขุมเงิน อำเภอเมือง จังหวัดยโสธร จำนวน 71 คน ดำเนินการวิจัยระหว่างเดือนพฤษภาคม – เดือนตุลาคม พ.ศ.2567 รวบรวมข้อมูลเชิงปริมาณและเชิงคุณภาพ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และเปรียบเทียบผลต่างค่าเฉลี่ยของคะแนนก่อนและหลังใช้สถิติ Paired Sample t-test กำหนดระดับนัยสำคัญที่ 0.05

ผลการศึกษา พบว่า กระบวนการพัฒนาประกอบด้วย การศึกษาสถานการณ์และบริบทพื้นที่ การวางแผนเชิงปฏิบัติการ การปฏิบัติตามแผน การสังเกตติดตาม สรุปผลและถอดบทเรียน ดำเนินการพัฒนารูปแบบการดำเนินงานป้องกันโรคไวรัสตับอักเสบบีในตำบลขุมเงิน อำเภอเมือง จังหวัดยโสธร ประกอบด้วย 4 กิจกรรม คือ 1) การคัดกรองด้วยแบบสอบถามและการตรวจเลือดที่ปลายนิ้ว 2) การจัดให้มีระบบการดูแลรักษาและส่งต่อกลุ่มผู้ติดเชื้อไวรัสตับอักเสบบี 3) การจัดให้มีระบบการเข้าถึงวัคซีนในกลุ่มผู้ที่ยังไม่เคยได้รับวัคซีนป้องกันโรคไวรัสตับอักเสบบี และ 4) การมีส่วนร่วมของชุมชน เช่น ผู้ใหญ่บ้าน เจ้าอาวาส ครู อาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม.) เพื่อช่วยในการประชาสัมพันธ์และให้ข้อมูลแก่ประชาชนในระดับครัวเรือนในการป้องกันโรคไวรัสตับอักเสบบี ภายหลังการดำเนินงานตามรูปแบบที่พัฒนาขึ้น พบว่า ค่าเฉลี่ยของคะแนนความรู้เกี่ยวกับโรคไวรัสตับอักเสบบีและความพึงพอใจ เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติเมื่อเปรียบเทียบกับก่อนดำเนินกิจกรรม (p < 0.05) รวมทั้ง ตรวจพบผู้ติดเชื้อไวรัสตับอักเสบบี จำนวน 3 ราย จากการคัดกรองด้วยการเจาะเลือดที่ปลายนิ้วและได้รับการส่งต่อเข้าสู่ระบบการรักษาที่เหมาะสม ซึ่งปัจจัยแห่งความสำเร็จในครั้งนี้ คือ การมีส่วนร่วมของภาคีเครือข่าย การคัดกรองและติดตามผลที่มีประสิทธิภาพ การส่งเสริมความรู้เกี่ยวกับการป้องกันโรคไวรัสตับอักเสบบี และการสนับสนุนจากผู้บริหารและหน่วยงานที่เกี่ยวข้อง ส่งผลให้การดำเนินงานเป็นไปอย่างเป็นระบบและนำไปสู่การพัฒนาที่ยั่งยืน
