Service delivery model using the telemedicine system in the clinic for elderly people with dementia at Nong Han Hospital Udonthani Province

Abstract

This research is an action research to study the service delivery model using the telemedicine system in the clinic for elderly people with dementia at Nong Han Hospital Udonthani Province. The target group consisted of 64 participants. Data were collected using questionnaires and participatory observation. The study was conducted from June 28, 2024, to September 2024.

The implementation of the telemedicine service model involved the use of the MorProm Station system, connecting service providers at the Dementia clinic of Nong Han Hospital with elderly patients with dementia. These patients attended scheduled appointments at subdistrict health promoting hospitals for symptom monitoring through the telemedicine system. The findings revealed that the service model improved cognitive performance. However, the cognitive abilities of participants still fell within the range of mild cognitive impairment or dementia. Although the average cognitive function scores increased, they remained abnormal overall. Additionally, scores for the ability to perform daily activities improved but showed that only 65.60% of participants reached a level of independence. Satisfaction with the telemedicine service model among elderly patients with dementia also increased.

The success factors of the telemedicine service model can be divided into five key areas : 1) technology readiness, 2) medical staff preparedness, 3) coordination among healthcare teams, 4) support systems, 5) evaluation and development to achieve goals. The success process involved planning, knowledge sharing, collaborative analysis, follow-up evaluations, and summarization. Recommendations suggest that continued use of the telemedicine service model in dementia clinics helps raise awareness among patients, boosts their morale, and ensures consistent monitoring of their conditions, enabling them to perform daily activities more effectively.

การวิจัยครั้งนี้เป็นการวิจัยเชิงปฏิบัติการ (Action Research) เพื่อศึกษารูปแบบการให้บริการด้วยระบบการแพทย์ทางไกลในคลินิกผู้สูงอายุที่มีภาวะสมองเสื่อม โรงพยาบาลหนองหาน จังหวัดอุดรธานี กลุ่มเป้าหมายจำนวนทั้งสิ้น 64 คน เก็บรวบรวมข้อมูลโดยใช้แบบสอบถาม และแบบสังเกตการณ์การมีส่วนร่วม ดำเนินการศึกษาระหว่างวันที่ 28 มิถุนายน 2567 - เดือนกันยายน 2567

จากการดำเนินงาน รูปแบบการให้บริการด้วยระบบการแพทย์ทางไกลในคลินิกผู้สูงอายุที่มีภาวะสมองเสื่อม โดยใช้ระบบหมอพร้อมสเตชั่นระหว่างผู้ให้บริการในคลินิกผู้สูงอายุโรงพยาบาลหนองหานและผู้รับบริการเป็นผู้สูงอายุที่มีภาวะสมองเสื่อมมารับบริการตามนัดหมายที่โรงพยาบาลส่งเสริมสุขภาพตำบล เพื่อติดตามอาการผ่านระบบการแพทย์ทางไกลนั้น พบว่า รูปแบบการให้บริการนี้ ส่งผลทำให้สมรรถภาพสมองดีขึ้นแต่ยังพบว่าระดับสมรรถภาพสมองยังอยู่ในระดับการรับรู้ทางเชาว์ปัญญาบกพร่องหรือมีภาวะสมองเสื่อม คะแนนเฉลี่ยพุทธิปัญญาเพิ่มขึ้นแต่ยังอยู่ในระดับผิดปกติทั้งหมด และความสามารถในการประกอบกิจวัตรประจำวันมีค่าคะแนนเฉลี่ยดีขึ้นแต่ก็ยังพบว่ายังอยู่ในระดับไม่พึ่งพาเพียงร้อยละ 65.60 และยังพบว่า ระดับความพึงพอใจจากการศึกษารูปแบบการให้บริการด้วยระบบการแพทย์ทางไกล ส่งผลต่อความพึงพอใจของผู้สูงอายุที่มีภาวะสมองเสื่อมเพิ่มขึ้น

ปัจจัยแห่งความสำเร็จ สามารถแบ่งออกเป็น 5 ด้าน ดังนี้ 1) ความพร้อมของเทคโนโลยี 2) ความพร้อมของบุคลากรทางการแพทย์ 3) การประสานงานระหว่างทีมดูแลสุขภาพ 4) การสนับสนุน 5) การประเมินผลและพัฒนาเพื่อให้บรรลุเป้าหมาย ตั้งแต่กระบวนการวางแผนแลกเปลี่ยนเรียนรู้แสดงความคิดเห็น วิเคราะห์ร่วมกัน มีการติดตามประเมินผล และสรุปผล ข้อเสนอแนะ การใช้รูปแบบการให้บริการด้วยระบบการแพทย์ทางไกลในคลินิกผู้สูงอายุที่มีภาวะสมองเสื่อมอย่างต่อเนื่องทำให้ผู้รับบริการตระหนัก มีกำลังใจและได้รับการติดตามอาการป่วยได้ต่อเนื่องสามารถปฏิบัติกิจวัตรประจำวันได้
