Preparatory process for preparing an integrated annual budget of the Subdistrict Health Promoting Hospital under the Maha Sarakham Provincial Administrative Organization.

Abstract

This participatory action research aimed to exploring the preparation process for the development of participation annual expenditure budget for Subdistrict health promotion hospitals (SHPH) under the Mahasarakham Provincial Administrative Organization (PAO). The study targeted were selected by purposive sampling and divided into two groups: 1) Those responsible for preparing budget proposals or those assigned with related tasks, totaling 33 individuals, and 2) Those responsible for the budget plan of SHPHs, totaling 121 individuals. The research process consisted of four stages: 1) Planning stage, 2) Action stage, 3) Observation stage, and 4) Reflection stage. Data were collected through focus group discussions and questionnaires. Quantitative data were analyzed using descriptive statistics and inferential statistics was Wilcoxon Matched-pairs Signed Rank Test, while qualitative data were analyzed using content analysis. The results revealed that prior to the process implementation, the context of budget preparation faced issues such as a lack of data, knowledge, and unclear procedures, communication and coordination were not continuous, leading to the planning of solutions. The development process for budget preparation involved creating a manual, setting a timeline for implementation, increasing communication channels, forming a working group, providing training activities, and developing flowcharts for the operations of each unit. The results also showed that after the development, those responsible for the plans and budgets were more mean score of knowledge, role perception, and participation than before development and after the development, the mean score of satisfied toward preparation process has a statistically significant difference at the 0.05 level. In summary, the process for preparing the annual expenditure budget for SHPHs consists of defining goals and objectives, collecting estimated data and assumptions based on each SHPH’s mission plan, preparing models for overall responsibility to adjustments, and drafting documents to be submitted to the PAO's Strategy and Budget Division. The success factors identified in this study include the involvement of all organizational levels, periodic data monitoring, and consistent coordination.

การศึกษาครั้งนี้ เป็นการวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม มีวัตถุประสงค์เพื่อศึกษากระบวนการเตรียมความพร้อมในการจัดทำงบประมาณรายจ่ายประจำปีแบบบูรณาการของโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) สังกัดองค์การบริหารส่วนจังหวัดมหาสารคาม (อบจ.) กลุ่มเป้าหมายเลือกแบบเฉพาะเจาะจง แบ่งเป็น 2 กลุ่ม คือ 1) ผู้รับผิดชอบจัดทำคำของบประมาณหรือผู้ที่ได้รับมอบหมาย และผู้เกี่ยวข้อง จำนวน 33 คน และ 2) ผู้รับผิดชอบแผนงบประมาณ รพ.สต. จำนวน 121 คน กระบวนการวิจัยมี 4 ขั้นตอน ประกอบด้วย 1) ขั้นวางแผน 2) ขั้นปฏิบัติการ 3) ขั้นการสังเกต และ 4) ขั้นการสะท้อนผล โดยเก็บรวบรวมข้อมูลจากการสนทนากลุ่ม และแบบสอบถาม วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติเชิงพรรณนาและสถิติเชิงอนุมาน ได้แก่ Wilcoxon Matched-pairs Signed Rank และข้อมูลเชิงคุณภาพวิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหา ผลการศึกษาบริบทของการจัดเตรียมงบประมาณก่อนดำเนินการพบว่า มีปัญหายังขาดข้อมูล ความรู้ ขั้นตอนแนวทาง การประสาน งานยังไม่ต่อเนื่อง การสื่อสารยังไม่ชัดเจนจึงนำไปสู่การวางแผนเพื่อแก้ปัญหา ผลการศึกษากระบวนการเตรียมความพร้อมในการจัดทำงบประมาณตามแผน พบว่า มีการพัฒนาโดยจัดทำคู่มือ กำหนดช่วงเวลาในการดำเนินการ มีการเพิ่มช่องทางสื่อสาร จัดตั้งคณะทำงาน มีกิจกรรมให้ความรู้และจัดทำแผนภูมิผังการดำเนินงานของแต่ละหน่วยงาน ผลการศึกษาผลของการพัฒนากระบวนการเตรียมความพร้อมพบว่าหลังการพัฒนาผู้รับผิดชอบแผนและงบประมาณพบว่าคะแนนเฉลี่ยความรู้ การรับรู้บทบาท และการมีส่วนร่วมเพิ่มขึ้น โดยมีคะแนนเฉลี่ยความพึงพอใจต่อกระบวนการเตรียมความพร้อมโดยมีความแตกต่างจากก่อนพัฒนาอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 โดยสรุปกระบวนการเตรียมความพร้อมในการจัดทำงบประมาณรายจ่ายประจำปีแบบบูรณาการของ รพ.สต.ประกอบด้วย การมีแนวทางกำหนดเป้าหมายและวัตถุประสงค์หลัก การรวบรวมข้อมูลประมาณการและสมมติฐานตามแผนภารกิจหน่วยงานแต่ละด้านการเตรียมแบบจําลองให้ผู้รับผิดชอบโดยรวมได้ปรับเปลี่ยนและการมีร่างเอกสารข้อมูลส่งให้กองยุทธศาสตร์และงบประมาณ ของ อบจ. ซึ่งการศึกษาครั้งนี้พบปัจจัยแห่งความสำเร็จ คือ การมีส่วนร่วมของทุกระดับในองค์กร มีการติดตามข้อมูลเป็นระยะ และการประสานงานกันอย่างสม่ำเสมอ
