Developing a System for Caring for Patients among Uncontrolled Hypertension Patients, through the Participation of Families and Communities. Na Mueang Subdistrict, Koh Samui District,Surat Thani Province

Abstract

This participatory action research aimed to develop a care system for patients with uncontrolled hypertension through the involvement of families and communities in Namuang Subdistrict, Koh Samui District, Surat Thani Province. The study participants were divided into three groups: 108 patients with hypertension, 108 primary caregivers from the patients’ households, and 25 healthcare service providers. Data were collected through in-depth interviews, focus group discussions, observations, participatory meetings, and questionnaires. Quantitative data were analyzed using descriptive statistics and paired samples t-test, while qualitative data were analyzed through content analysis. The Chronic Care Model (CCM) was applied in the development process.

The results revealed that the development process of the care system for patients with uncontrolled hypertension through family and community participation comprised four phases: (1) action plan meeting, (2) implementation of the plan, (3) monitoring and observing participation, and (4) conclusion and lesson learned. This led to the creation of the “NAMUANG MODEL” consisting of seven components 1) N (NCD Care Focused): Emphasis on the care of non-communicable diseases (NCDs) 2) A (Affiliated with Community): Strong collaboration with the community 3) M (Managing Local Resources): Self-management and effective utilization of local resources 4) U (Up Skills and Empowerment): Capacity building and empowerment of personnel and the public 5) A (Adapting Care Service): Adaptation of health services to suit local needs 6) N (Network via Technology): Integration of health networks through technology 7) G (Good Governance): Upholding principles of good governance, transparency, accountability, and inclusive participation from all sectors.

Key success factors included 1) Strong network partnerships, administrative support, and systematic collaboration among multidisciplinary teams; 2) Planning that aligned with community contexts and needs; 3) Close support from families and communities in patient care; 4) Improved access to healthcare services through various service channels; 5) Effective use of information technology in communication with healthcare providers, enhancing the coverage and efficiency of the care system.

การวิจัยนี้เป็นการวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม มีวัตถุประสงค์เพื่อการพัฒนาระบบการดูแลผู้ป่วยโรคความดันโลหิตสูงที่ควบคุมไม่ได้ โดยการมีส่วนร่วมของครอบครัวและชุมชน ตำบลหน้าเมือง อำเภอเกาะสมุย จังหวัดสุราษฎร์ธานี กลุ่มตัวอย่างในการวิจัยแบ่งออกเป็น 3 กลุ่ม ได้แก่ ผู้ป่วยโรคความดันโลหิตสูง จำนวน 108 คน ผู้ดูแลป่วยโรคความดันโลหิตสูงประจำครัวเรือน จำนวน 108 คน และผู้จัดรูปแบบบริการสุขภาพ จำนวน 25 คน เก็บรวมรวมข้อมูลโดยการสัมภาษณ์เชิงลึก การสนทนากลุ่ม การสังเกต การประชุมแลกเปลี่ยนเรียนรู้และการใช้แบบสอบถาม วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติเชิงพรรณนา และ Paired Samples t-test วิเคราะห์ข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เชิงเนื้อหา ประยุกต์ใช้ตัวแบบการจัดการโรคเรื้อรัง (Chronic Care Model)

ผลการวิจัย พบว่า กระบวนการพัฒนาระบบการดูแลผู้ป่วยโรคความดันโลหิตสูงที่ควบคุมไม่ได้ โดยการมีส่วนร่วมของครอบครัวและชุมชน ประกอบด้วย 4 ขั้นตอน คือ 1) ประชุมจัดทำแผนปฏิบัติการ 2) ปฏิบัติตามแผน 3) ติดตามผล สังเกตการมีส่วนร่วม 4) สรุปผลและถอดบทเรียน ทำให้เกิดแนวคิดการพัฒนาระบบการดูแลผู้ป่วยโรคความดันโลหิตสูง ภายใต้ระบบ NAMUANG MODEL มี 7องค์ประกอบ ดังนี้ 1) N (NCD Care Focused) เน้นการดูแลโรคไม่ติดต่อเรื้อรัง (NCDs) 2) A (Affiliated with Community) มีความร่วมมือกับชุมชน 3) M (Managing Local Resources) จัดการตนเองและบริหารทรัพยากรในพื้นที่ 4) U (Up Skills and Empowerment) ยกระดับทักษะเสริมพลังบุคลากรและประชาชน 5) A (Adapting Care Service) ปรับระบบการให้บริการสุขภาพให้เหมาะสม 6) N(Network via Technology) เชื่อมโยงเครือข่ายการดูแลสุขภาพด้วยเทคโนโลยี 7) G (Good Governance) ยึดหลักธรรมาภิบาล ตรวจสอบได้ มีความรับผิดชอบ และมุ่งเน้นการมีส่วนร่วมจากทุกภาคส่วนอย่างเท่าเทียม

ปัจจัยแห่งความสำเร็จประกอบด้วย 1) ภาคีเครือข่ายที่เข้มแข็ง การสนับสนุนจากผู้บริหาร และการทำงานร่วมกันอย่างเป็นระบบของทีมสหสาขาวิชาชีพ 2) มีการวางแผนที่สอดคล้องกับบริบทและความต้องการของชุมชน 3) ได้รับการสนับสนุนอย่างใกล้ชิดจากครอบครัวและชุมชนในการดูแลผู้ป่วย 4) ผู้ป่วยสามารถเข้าถึงบริการสุขภาพได้สะดวกยิ่งขึ้น ผ่านช่องทางการให้บริการที่หลากหลาย 5) การใช้เทคโนโลยีสารสนเทศในการติดต่อสื่อสารกับบุคลากรทางการแพทย์ส่งผลให้ระบบการดูแลผู้ป่วยมีความครอบคลุมและมีประสิทธิภาพมากยิ่งขึ้น
