Tuberculosis in Correctional Facilities: A Cross-Sectional Study of Knowledge, Attitudes, and Practices of Prison Health Volunteer

Abstract

Tuberculosis persists as a challenge in prisons, with ineffective control risking further transmission and poor health outcomes. This study examines the knowledge, attitudes, and roles of prison volunteer health workers (VHWs) in the surveillance, prevention, and control of tuberculosis (TB). Design Cross-sectional study. Additionally, it seeks to identify factors influencing the effectiveness of their efforts. Participants Data were collected from 298 VHVs between March and July 2023. Primary and secondary outcomes. The knowledge, attitudes, and roles of VHWs in TB control were analyzed using descriptive statistics, and factors influencing them were analyzed using multiple logistic regression. Results among 298 participants, 94.6% of volunteer health workers (VHWs) needed more knowledge. Most VHWs showed moderate (77.5%) to low (17.1%) attitudes toward TB control, with 63.4% performing their roles adequately and 19.5% at a moderate level. Key factors significantly influencing VHW performance were their attitude toward the role (P < 0.01; adj. OR = 3.73, 95% CI: 1.21–12.39) and having received training (P < 0.01; adj. OR = 2.94, 95% CI: 1.33–6.47). Conclusion: Volunteer health workers' (VHWs) effectiveness in TB control is influenced by their attitudes and training. Strengthening VHWs' knowledge through targeted training programs is essential to enhance their performance and improve TB management in correctional facilities.

วัณโรคยังคงเป็นปัญหาในเรือนจำ การแพร่ระบาดของวัณโรคย่อมส่งผลกระทบต่อสุขภาพผู้ต้องขัง การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาระดับความรู้ ทัศนคติและบทบาทของอาสาสมัครสาธารณสุขเรือนจำ (อสรจ.) ในการดำเนินงานเฝ้าระวัง และควบคุมวัณโรค เป็นการศึกษาแบบภาคตัดขวางเชิงวิเคราะห์ โดยเก็บข้อมูลจากอาสาสมัครสาธารณสุขเจือนจำ (อสรจ.) จำนวน 298 คน ในทัณฑสถานวัยหนุ่มทั้ง 3 แห่ง คือ ทัณฑสถานวัยหนุ่มกลาง ทัณฑสถานวัยหนุ่มพระนครศรีอยุธยา ทัณฑสถานวัยหนุ่มนครศรีธรรมราช ซึ่งดำเนินการในช่วงเดือนมีนาคม 2567 – กรกฎาคม 2567 รวบรวมข้อมูลโดยใช้แบบสอบถาม ได้แก่ แบบสอบถามข้อมูลทั่วไป ความรู้เรื่องวัณโรค ทัศนคติที่มีต่อการควบคุมวัณโรค การปฏิบัติงานตามแนวทางการควบคุมวัณโรคของ อสรจ. โดยที่เนื้อหาครอบคลุมวัตถุประสงค์การวิจัยและผ่านการตรวจสอบคุณภาพ และความเหมาะสมโดยผู้เชี่ยวชาญ ทดสอบความเชื่อถือและปรับปรุงวิเคราะห์หาความถูกต้องโดยใช้ Cronbach’s alpha coefficient of reliability ได้ค่า 0.948 และทดสอบความเชื่อมั่นโยใช้วิธีการของคูเดอร์ ริชาร์ดสัน แบบ KR20 ได้ค่า 0.641 วิเคราะห์ข้อมูลเชิงพรรณนา แสดงค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และวิเคราะห์หาความสัมพันธ์ด้วย Chi-square test และในกรณีความถี่ของตัวแปรที่ศึกษาน้อยกว่าความถี่ที่คาดหวังใช้สถิติ Fisher’s exact test และในการทดสอบปัจจัยที่มีผลต่อประสิทธิผลการดำเนินงานควบคุมวัณโรคโดย อสรจ. ใช้วิเคราะห์การถดถอยพหุคูณ (Multiple linear regression) ผลการศึกษาพบว่า ความรู้อยู่ในระดับต่ำ (ร้อยละ 94.6) ระดับทัศนคติที่มีต่อการควบคุมวัณโรค อยู่ในระดับปานกลาง (ร้อยละ 77.5) และต่ำ (17.1%) การปฏิบัติงานตามบทบาทของ อสรจ.ได้ดี (ร้อยละ 63.4) ส่วนด้านความสัมพันธ์พบว่า ปัจจัยที่มีความสัมพันธ์กับการแสดงบทบาทของ อสรจ.ในการเฝ้าระวังป้องกันควบคุมวัณโรคอย่างมีนัยสำคัญทางสถิติ P val.
