The Integrated Community Care Model for Diabetes mellitus patient group, Warinchamrab District, Ubon Ratchathani Province

Abstract

This action research aimed to develop the care model for patients with type 2 diabetes by Integrated with community care in the area of public health service center 3, Warinchamrab District, Ubon Ratchathani Province. The 97 persons of target group were selected. Such as patients with type 2 diabetes of 30 persons, Volunteers care of 30 persons, Municipal district agent, public health officer and community leaders agent of 37 persons. Collected both Qualitative data and Quantitative data. The tools of research were composed of the in-depth interviews and structured questionnaire, focus groups, interviews and participated meeting. Statistics used for data analysis included descriptive statistics. Qualitative data were analyzed by content analysis.

The results showed that this development of care model for patients with type2 diabetes consisted 9 stages: 1) data collection 2) NCD board team 3) action plan 4) development of volunteer capacity 5) training on the job and health behavior modification 6) home visits diabetes patients 7) evaluation practice 8) educational exchange and 9) after action review.The target group had a higher level of knowledge, satisfaction and participation. The diabetic patients could control blood sugar levels to a great extent 66.67%. The model discovered in this operation is called “VARIN Model” (V: Volunteers, A: Area Base, R: Reinforcement, I: Integrated care and N: Network)

In conclusion, The supporting factors for succeed of this research were the creating a volunteer spirit and reinforcement positive forces for the target group on area base according to holistic care by interprofessional team and volunteer groups with strong and creative coordination.

การวิจัยเชิงปฏิบัติการนี้ มีวัตถุประสงค์เพื่อพัฒนารูปแบบการดูแลผู้ป่วยเบาหวานชนิดที่ 2 โดยบูรณาการความร่วมมือในชุมชน ในพื้นที่ศูนย์บริการสาธารณสุขแห่งที่ 3 อำเภอวารินชำราบ จังหวัดอุบลราชธานี กลุ่มเป้าหมายทั้งหมด 97 คน ประกอบด้วย กลุ่มผู้ป่วยเบาหวาน 30 คน, จิตอาสา 30 คน และกลุ่มตัวแทนจากเทศบาล, เจ้าหน้าที่สาธารณสุขและตัวแทนชุมชน จำนวน 37 คน เก็บรวบรวมข้อมูลเชิงปริมาณและเชิงคุณภาพ เครื่องมือที่ใช้ในการเก็บข้อมูลเชิงปริมาณคือแบบสอบถามที่สร้างขึ้น และเก็บรวบรวมข้อมูลเชิงคุณภาพโดย การสนทนากลุ่ม การสัมภาษณ์ และการประชุมแบบมีส่วนร่วม วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติเชิงพรรณนา วิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์ข้อมูลเชิงเนื้อหา (Content Analysis)

ผลการวิจัยพบว่า การพัฒนาครั้งนี้มี 9 ขั้นตอน ได้แก่ 1) การรวบรวมข้อมูล 2) จัดตั้งคณะกรรมการ 3) การจัดทำแผนปฏิบัติการ 4) การพัฒนาศักยภาพจิตอาสา 5) การให้ความรู้และการปรับเปลี่ยนพฤติกรรม 6) การติดตามเยี่ยมบ้าน 7) การนิเทศติดตามและสนับสนุน 8) การจัดเวทีแลกเปลี่ยนเรียนรู้ และ 9) การถอดบทเรียน ผลของกระบวนการดังกล่าวส่งผลให้กลุ่มเป้าหมาย มีการเปลี่ยนแปลงระดับความรู้ ความพึงพอใจ และระดับการมีส่วนร่วม เพิ่มขึ้น กระบวนการนี้ ส่งผลให้มีจำนวนผู้ป่วยที่สามารถควบคุมระดับน้ำตาลในเลือดได้ดีขึ้น มีจำนวนเพิ่มจากเดิมคิดเป็นร้อยละ 66.67 รูปแบบที่ค้นพบในการดำเนินงานครั้งนี้เรียกว่า VARIN Model ซึ่งประกอบด้วย การสร้างจิตอาสา การพัฒนาพื้นที่ มีพลังเสริม เพิ่มเติมบริการ ประสานงานชุมชน

โดยสรุปปัจจัยแห่งความสำเร็จของการศึกษาในครั้งนี้คือ การสร้างจิตอาสาและการเสริมพลังเชิงบวกให้กับกลุ่มเป้าหมาย โดยเน้นการทำงานในพื้นที่เป็นฐานสำคัญ ด้วยการจัดให้บริการแบบองค์รวม โดยทีมสหวิชาชีพและกลุ่มจิตอาสาที่มีการประสานงานกันที่เข้มแข็งและสร้างสรรค์
