The Development of Long-Term Care Systems for the Dependent Elderly in Nacharoen Sub-district Det-Udom District Ubonratchathani Province

Abstract

This study was a participatory action research to study the situation of the dependent elderly care in Nacharoen Sub-district and to develop a long-term care system for dependent elderly in Nacharoen Sub-district, Det-Udom District, Ubonratchathani Province. The 210 participants were selected according to the specified criteria namely academic department consisting of public health academics, the local sector consists of a subcommittee to support long-term care for the dependent elderly, committee of the quality of life development center for older persons in the community, And the public sector consisted of the dependent elderly, the dependent elderly caregivers and village health volunteers. Data were collected by using the observation form participatory survey, questionnaire, recorded discussion and the data analysis group using descriptive statistics including percentage, mean and standard deviation.

The results of the study showed that the care for the dependent elderly in Nacharoen Sub-district was started in 2018. Mainly through the co-operation of the academic and local departments. But the public sector has unclear co-operation role. When assessing readiness and creating a process for developing a long-term care system for the dependent elderly in Nacharoen Sub-district, Det-Udom District, Ubonratchathani Province in 4 steps : 1) Studying context data, analyze the problem of needs of the dependent elderly, appoint a subcommittee to support the long-term care for the dependent elderly and committee of the quality of life development center for older persons in Nacharoen Sub-district for a workshop planning meeting to define activities and projects to organize long-term care services for the dependent elderly in Nacharoen Sub-district, Det-Udom District, Ubonratchathani Province. 2) Action to the project according to the established process with the participation of all sectors according to their roles and responsibilities. 3) All sectors participate in supervise, monitor, evaluate and support the implementation of all activity to their roles and responsibilities. 4) All sectors participate in summarizing the results of operations and making recommendations for further development. In doing so, the dependent elderly in Nacharoen Sub-district are cared by the dependent elderly long-term care system fund. The dependent elderly, caregivers and stakeholders were satisfied with the development of long-term care system for the dependent elderly. And the long-term care systems for the dependent elderly in Nacharoen Sub-district, Det-Udom District, Ubonratchathani Province can passed the long-term care all standard certification from Regional Health Promotion Center 10 Ubonratchathani.

The success factors are the co-operation and support from all sectors. This co-operation making the process of development of long-term care system for the dependent elderly has been the long-term care standard certification and continuity, suitable for the context area of ​​Nacharoen Sub-district, Det-Udom District, Ubonratchathani Province.

การศึกษาครั้งนี้ เป็นการวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม เพื่อศึกษาสถานการณ์การดูแลผู้สูงอายุที่มีภาวะพึ่งพิงในพื้นที่ตำบลนาเจริญ และพัฒนาระบบการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิงในพื้นที่ตำบลนาเจริญ อำเภอเดชอุดม จังหวัดอุบลราชธานี ผู้เข้าร่วมวิจัยเลือกตามเกณฑ์ที่กำหนด จำนวน 210 คน คือ ภาควิชาการ ประกอบด้วย นักวิชาการสาธารณสุข ภาคท้องถิ่น ประกอบด้วย คณะอนุกรรมการสนับสนุนการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิง คณะกรรมการศูนย์พัฒนาคุณภาพชีวิตผู้สูงอายุในชุมชน และภาคประชาชน ประกอบด้วย ผู้สูงอายุที่มีภาวะพึ่งพิง ผู้ดูแลช่วยเหลือผู้สูงอายุที่มีภาวะพึ่งพิง ผู้ดูแลผู้สูงอายุที่มีภาวะพึ่งพิงประจำครัวเรือน อาสาสมัครสาธารณสุขประจำหมู่บ้าน เครื่องมือที่ใช้เก็บข้อมูลเป็นแบบสอบถาม แบบสัมภาษณ์ และแบบบันทึกการประชุม การสนทนากลุ่ม สถิติที่ใช้วิเคราะห์ข้อมูล ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน

ผลการศึกษา พบว่า การดูแลผู้สูงอายุที่มีภาวะพึ่งพิงในพื้นที่ตำบลนาเจริญ ได้เริ่มดำเนินการในปี 2561 โดยความร่วมมือของภาควิชาการ และภาคท้องถิ่นเป็นหลัก แต่ภาคประชาชนยังไม่มีบทบาทร่วมดำเนินการชัดเจน เมื่อประเมินความพร้อมและสร้างกระบวนการพัฒนาระบบการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิงในพื้นที่ตำบลนาเจริญ อำเภอเดชอุดม จังหวัดอุบลราชธานี 4 ขั้น คือ 1) ศึกษาบริบท วิเคราะห์ปัญหา ความต้องการของผู้สูงอายุที่มีภาวะพึ่งพิง แต่งตั้งคณะอนุกรรมการสนับสนุนการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิง และคณะกรรมการศูนย์พัฒนาคุณภาพชีวิตผู้สูงอายุตำบลนาเจริญ เพื่อประชุมวางแผนเชิงปฏิบัติการ กำหนดกิจกรรมและโครงการจัดบริการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิงในพื้นที่ตำบลนาเจริญ อำเภอเดชอุดม จังหวัดอุบลราชธานี 2) ดำเนินโครงการตามกระบวนการที่ได้กำหนด โดยการมีส่วนร่วมของทุกภาคส่วนตามบทบาทหน้าที่ 3) ทุกภาคส่วนร่วมกำกับ ติดตาม ประเมินผล และสนับสนุนการดำเนินงานทุกกิจกรรมตามบทบาทหน้าที่ 4) ทุกภาคส่วนร่วมสรุปผลการดำเนินงาน และเสนอแนะแนวทางเพื่อการพัฒนาต่อไป ซึ่งการดำเนินงานดังกล่าวทำให้ผู้สูงอายุที่มีภาวะพึ่งพิงในพื้นที่ตำบลนาเจริญได้รับการดูแลจากกองทุนระบบการดูแลระยะยาวสำหรับผู้สูงอายุที่มาภาวะพึ่งพิง ผู้สูงอายุที่มีภาวะพึ่งพิง ญาติผู้ดูแลและผู้มีส่วนเกี่ยวข้องมีความพึงพอใจต่อการพัฒนาระบบการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิง และระบบการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิงในพื้นที่ตำบลนาเจริญ อำเภอเดชอุดม จังหวัดอุบลราชธานี ผ่านการรับรองตำบลดูแลผู้สูงอายุระยะยาว (Long Term Care : LTC) ศูนย์อนามัยที่ 10 กรมอนามัย จังหวัดอุบลราชธานี ทุกมาตรฐาน

ปัจจัยแห่งความสำเร็จ คือ การได้รับความร่วมมือ และการสนับสนุนจากทุกภาคส่วน จึงทำให้การดำเนินกระบวนการพัฒนาระบบการดูแลระยะยาวสำหรับผู้สูงอายุที่มีภาวะพึ่งพิง มีการดำเนินการที่ผ่านเกณฑ์มาตรฐาน มีความต่อเนื่อง เหมาะสมกับบริบทของพื้นที่ตำบลนาเจริญ อำเภอเดชอุดม จังหวัดอุบลราชธานี
