The Quality Management Model of Primary Care Custer by Community Participation Mechanism for Primary Care Custer Standard Si Songkhram Sub-District Amphoe Wang Saphung Loei Province

Abstract

This is an action research. The objectives of the research were to study The Quality Management Model of Primary Care Cluster (PCC) by Community Participation Mechanism for Primary Care Cluster Standard at Non Sawang Health Promoting Hospital,Si Songkhram Sub-District ,Wang Saphung District, Loei Province. The target group are those involved in Primary Care Cluster by order to appoint a Primary Care Cluster committee, consisting of 5 political parties, 29 academic departments and 16 public sector, a total of 50 people, collecting both quantitative and qualitative data. Data were analyzed by using percentage, mean, standard deviation. Compared between before and after development of the model with Paired t-test dependent statistic and content analysis.

The results showed that The development process has 11 steps consisting of 1) Establishing a Primary Care Cluster team 2) Studying the context of the Primary Care Cluster 3) Analyzing issues, identifying problems, finding real causes And set goals for joint planning of operations. 4) Select solutions to problems, develop administrative systems, Organize Primary Care Cluster Establish an action plan. 5) Assess knowledge and understanding about Primary Care Cluster operations. Operational satisfaction and participation in operations (before) 6) Workshop on the Family Doctor Clinic criteria (3S) 7) Follow the Primary Care Cluster guidelines 8) Supervision, follow up / support, Observe operations. 9) Summary of issues and assess operations. 10) Assess knowledge and understanding about Primary Care Cluster operations. Operational satisfaction and participation in operations (after) 11) Summary of operations and take lesson learned; according to the model, the success, the problems, the obstacles of operations and the development opportunities. The results of the development model were that found the knowledge and understanding about Primary Care Cluster operations , the operational satisfaction and participation in operations of target group between before development were significantly different from after development with statistically significant different at 95% confidence level. This action research can give the model of development of a Quality Management Model of Primary Care Cluster by Community Participation Mechanism for Primary Care Cluster Standard at Non Sawang Health Promoting Hospital Si Songkhram Sub-District Wang Saphung District, Loei Province by Participation Mechanism for Primary Care Cluster (PCC).

Conclusion, the succeed of the development of a Quality Management Model of PCC by Community Participation Mechanism for to be Primary Care Cluster Standard Occurred by the participation mechanism of the Primary Care Cluster team from all of sector. Including; the participation decision, the Participatation in the operated working, the Participation in receiving benefits and the participatation in the evaluation.

การวิจัยเชิงปฏิบัติการครั้งนี้ มีวัตถุประสงค์เพื่อศึกษารูปแบบการจัดการคุณภาพคลินิกหมอครอบครัวโดยกลไกการมีส่วนร่วมของชุมชนเพื่อเป็นคลินิกหมอครอบครัวต้นแบบของโรงพยาบาลส่งเสริมสุขภาพตำบลโนนสว่าง ตำบลศรีสงคราม อำเภอวังสะพุง จังหวัดเลย กลุ่มเป้าหมาย คือ ผู้ที่มีส่วนเกี่ยวข้องในการจัดการคุณภาพของคลินิกหมอครอบครัวโดยคำสั่งแต่งตั้งคณะกรรมการคลินิกหมอครอบครัว ได้แก่ ภาคการเมือง จำนวน 5 คน ภาควิชาการ จำนวน 29 คน และภาคประชาชน จำนวน 16 คน รวมทั้งสิ้น 50 คน เก็บข้อมูลทั้งเชิงปริมาณและเชิงคุณภาพโดยแบบสอบถามที่สร้างขึ้น วิเคราะห์ข้อมูลโดยใช้ ค่าเฉลี่ย ร้อยละ ส่วนเบี่ยงเบนมาตรฐาน เปรียบเทียบก่อนและหลังการพัฒนาด้วย Paired t-test dependent และวิเคราะห์ข้อมูลเชิงเนื้อหา

ผลการวิจัยพบว่า กระบวนการพัฒนาครั้งนี้มี 11 ขั้นตอนประกอบด้วย 1) จัดตั้งทีมหมอครอบครัว 2) ศึกษาบริบทคลินิกหมอครอบครัว 3) วิเคราะห์ประเด็นปัญหา ระบุปัญหา หาสาเหตุแท้จริง และตั้งเป้าหมายร่วมกันวางแผนการดำเนินงาน 4) เลือกแนวทางแก้ปัญหาพัฒนาระบบบริหารจัดคลินิกหมอครอบครัว กำหนดแผนปฏิบัติการ 5) ประเมินความรู้ความเข้าใจเกี่ยวกับการดำเนินงานคลินิกหมอครอบครัว ความพึงพอใจในการดำเนินงาน และการมีส่วนร่วมในการดำเนินงาน (ก่อน) 6) ประชุมเชิงปฏิบัติการตามเกณฑ์คลินิกหมอครอบครัวต้นแบบ (3S) 7) ปฏิบัติตามแนวทางคลินิกหมอครอบครัว 8) นิเทศติดตาม/สนับสนุน สังเกตการดำเนินงาน 9) สรุปประเด็นและประเมินผลการดำเนินงาน 10) ประเมินความรู้ความเข้าใจเกี่ยวกับการดำเนินงานคลินิกหมอครอบครัว ความพึงพอใจในการดำเนินงาน และการมีส่วนร่วมในการดำเนินงาน (หลัง) 11) สรุปผลการดำเนินงาน ถอดบทเรียนตามรูปแบบ ความสำเร็จที่เกิดขึ้น ปัญหา อุปสรรคการดำเนินงานและโอกาสพัฒนา กระบวนการดังกล่าวส่งผลให้ความรู้ความเข้าใจเกี่ยวกับการดำเนินงานคลินิกหมอครอบครัว ความพึงพอใจในการดำเนินงาน และการมีส่วนร่วมในการดำเนินงาน ของกลุ่มเป้าหมายในระยะก่อนและหลังการพัฒนามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ ด้วยระดับความเชื่อมั่น 95% การดำเนินงานครั้งนี้ทำให้ได้รูปแบบเบื้องต้นในการพัฒนารูปแบบการจัดการคุณภาพคลินิกหมอครอบครัวโดยกลไกการมีส่วนร่วมของชุมชนเพื่อเป็นคลินิกหมอครอบครัวต้นแบบของโรงพยาบาลส่งเสริมสุขภาพตำบลโนนสว่าง ตำบลศรีสงคราม อำเภอวังสะพุง จังหวัดเลย โดยกลไกการมีส่วนร่วมของทีมคลินิกหมอครอบครัว

สรุป ความสำเร็จของการพัฒนารูปแบบการจัดการคุณภาพคลินิกหมอครอบครัวโดยกลไกการมีส่วนร่วมของชุมชนเพื่อเป็นคลินิกหมอครอบครัวต้นแบบของโรงพยาบาลส่งเสริมสุขภาพตำบลโนนสว่าง เกิดขึ้นได้จากกลไกการมีส่วนร่วมของทีมคลินิกหมอครอบครัวทุกภาคส่วน ทั้งการมีส่วนร่วมในการตัดสินใจ มีส่วนร่วมในการดำเนินการ มีส่วนร่วมในการรับผลประโยชน์ และมีส่วนร่วมในการประเมินผล
