Development of the Pharmaceutical Care Model for Patients with Renal Impairment at Medical Ward in Suwannaphum Hospital, Roi-Et

Abstract

Patients with impaired renal function refer to the pharmacokinetics. The objective of this research was to develop a pharmaceutical care model in patients with impaired renal function in the internal medicine ward. This research is an action research. The study was divided into 3 phases. Phase I examined the problem of drug use in patients with impaired renal function from electronic medical records Between October 1, 2016 and September 30, 2017, a total of 300 patients, Phase 2, developed a manual for dosage adjustments in patients with impaired renal function. and the development of pharmaceutical care model. Phase 3 carried out full pharmaceutical care for 40 patients. The results showed that 1) Drug interactions 2) Over dosage 3) Untreated indications 4) Failure to receive medication 5) Improper drug selection 6) Adverse drug reactions There were 16 problems (38.10%), 10 problems (23.81%), 5 problems (11.90 %) , 5 problems (11.90%), 3 problems ( 7.14 %) and 3 problems (7.14), respectively. ผู้ป่วยที่มีการทำงานของไตบกพร่องจะส่งต่อเภสัชจลนศาสตร์ในการใช้ยา ผู้วิจัยมีวัตถุประสงค์พัฒนารูปแบบการบริบาลทางเภสัชกรรมในผู้ป่วยที่มีการทำงานของไตบกพร่องบนหอผู้ป่วยอายุรกรรม การวิจัยนี้เป็นการวิจัยเชิงปฏิบัติการ แบ่งการศึกษาออกเป็น 3 ระยะ ระยะที่ 1 ศึกษาปัญหาการใช้ยาในผู้ป่วยที่มีการทำงานของไตบกพร่อง จากเวชระเบียนอิเล็กทรอนิกส์ ระหว่างวันที่ 1 ตุลาคม 2559 ถึง 30 กันยายน 2560 จำนวน 300 ราย ระยะที่ 2 พัฒนาคู่มือการปรับขนาดยาในผู้ป่วยที่มีการทำงานของไตบกพร่อง และการพัฒนารูปแบบการบริบาลทางเภสัชกรรม ระยะที่ 3 ดำเนินการบริบาลทางเภสัชกรรมเต็มรูปแบบ 40 ราย ผลการศึกษาพบว่า 1) เกิดอันตรกิริยาระหว่างยา 2) ได้รับยาที่ถูกต้องแต่ขนาดมากเกินไป 3) ไม่ได้รับยาที่ควรจะได้ 4) ไม่ได้รับยาตามแพทย์สั่ง 5) ได้รับที่ไม่เหมาะสม 6)เกิดอาการไม่พึงประสงค์จากการใช้ยา จำนวน 16 ปัญหา(ร้อยละ 38.10), 10 ปัญหา(ร้อยละ 23.81), 5 ปัญหา(ร้อยละ11.90), 5 ปัญหา(ร้อยละ 11.90), 3 ปัญหา(ร้อยละ7.14) และ 3 ปัญหา(ร้อยละ 7.14) ตามลำดับ โดยทั้ง 42 ปัญหาได้รับการแก้ไขและติดตามโดยเภสัชกรตลอดระยะเวลาที่อยู่ในโรงพยาบาล คิดเป็นร้อยละ 100 และปัญหาทั้งหมด แพทย์และ/หรือผู้ป่วยให้การยอมรับและปฏิบัติตามที่เภสัชกรแนะนำ คิดเป็นร้อยละ 100 ดังนั้นการบริบาลทางเภสัชกรรมโดยเภสัชกร ส่งผลให้ผู้ป่วยได้รับการรักษาที่มีมาตรฐาน ปลอดภัย และได้รับประโยชน์สูงสุดจากการใช้ยา
